For Christmas, my niece wants me to replace Josephina's head with a new one, because her hair is in bad shape.
Can I keep the old head for myself (do a downy dunk and put on another body?)
Poor Josephina - I hate to talk about her in such gruesome terms...

I don't think they're supposed to do that. I know the rule is that they will only replace a head that is the same head, so if you send the doll in headless they won't know what head to put on. You could put a note in there that you want the head back. I think someone said they did that with Lindsey. They sent the head back and the person sold it on ebay. I don't think they are supposed to do that, however.
Your best bet is to call American Girl and ask CS directly.

You need to send the whole doll in to the doll hospital, and put a note on it (I use a rubberband around the dolls' torso) telling them to replace the head, and return the old head to you. They will return it. It belongs to you, so by rights, they shouldn't keep it.
However, I sent a doll in for new torso and limbs, and they called and wanted to put a new head on the doll, because the vinyl color was different, and it wouldn't match. I asked if they would return the old head, and they said no, because I wasn't paying for a head replacement, they were doing it free.
In other words, if you are paying for a new head, they should return the old head if you ask.
Just make sure you put that request on your hospital admission form, and also on a seperate note attached to the doll.
